Question: My mother is 74 and very active and healthy but is on Plavix after having a moderate stroke (with not after effects). Today she got gout in her finger and her doctor told her she can't have painkillers but maybe Tylenol once in a while. That sound weird to me that she could have it "once in a while". It is either dangerous or it is not. She also has a 3mm aneurysm that was accidentally found on the stroke MRI so I don't want her to risk any bleeding. Is it OK to take Tylenol with Aspirin/Plavix and what are the best ways to relieve pain...heat or cold?

Brief Answer:
Yes you can take Tyelenol along with plavix by taking some precautions.

Detailed Answer:
Hello,

You can take Tyelenol along with Plavix whenever it is necessary for pain relief. Plavix is clopidogrel an antiplatelet medication for stroke patients. Tyelenol is paracetamol a pain killer. You have other medications such as NSAIDS, anti gout drugs like Colchicine which are more efficient pain relievers for gout. Most of the pain killers including Tyelenol can cause peptic ulcer bleed which is further aggravated by taking plavix. To prevent this you need to take Proton pump inhibitors as a precaution. For further more relief you need to apply ice on the painful area.I hope I have answered your question.

If you have further queries I'm happy to help you.
Thanks.

A couple more questions:
#1 Can high uric acid cause problems more serious than gout. She would like to treat this with diet instead of meds if the risk is only reoccurrence of gout.
#2 Can she drink tart cherry juice concentrate with Plavix and aspirin? Like I mentioned she has a 3 mm brain aneurysm and we are not sure if Plavix increases the risk of rupture so we are very cautious of adding anything blood thinning to her diet.
Thanks!

Brief Answer:
Yes you can tart cherry juice along with Plavix or aspirin but moderately

Detailed Answer:
Hi,

Thanks for writing back.

High uric acid levels can cause kidney damage due to deposition of uric acid apart from causing gout and arthritis but it's a late complications.Yiu can rule out renal damage with renal function tests and ultrasonography. You can control your body uric acid levels by following strict dietary habits but if uric acid levels are elevated considerably you have to use medications to decrease uric acid to prevent renal damage.

Some of foods that can cause high uric acid include red meat ,liver, beans, cauliflower and alcohol beverages.Tart cherry juice has lot of amazing health benefits along with giving pain relief from gout and arthritis. You can take this juice 250-300ml per day without side effects.Like NSAIDS it also has Cox inhibitor which can gastric mucosal ulceration leading to peptic ulcer disease but this side effect is only when taken in high quantities. You need to use proton pump inhibitors to prevent peptic ulcer which can be caused by plavix or aspirin too.

I hope I have answered your question. If you have further queries I'm happy to help you.
Thanks.

Thank you so much I appreciate it!
#1 So, the cherry juice is not blood thinning, correct? The reason
you said use moderation is concerning gastric issues?
#2 And what would be considered too high on a blood test to
go with diet restriction only. This is a first time flare up.
Thank you for the information has been a great help. It is hard
to get details during office visits it seems. I will rate

OK, so disregard all the previous questions the blood work
just came back and it is not gout. She is in extreme pain and
hasn't slept in 2 days. Is on Sulfa in case of infection. We are
going into the weekend and she is caregiving her 96 year old
mother and the doctor did not tell her what it might be instead!
Doesn't appear to be a bite. What else look and feels like gout?
Her upper finger is red, swollen, feverish and the nail is turning
blue. It came out of nowhere!!! Really confused now....

Brief Answer:
It could be due to osteoarthritis but other causes need to be ruled out.

Detailed Answer:
" Hello and Welcome to 'Ask A Doctor' service. I have reviewed your query and here is my advice." So gout has been ruled out and that's good news.Gout usually affects great toe first ,followed by other joints.Anyway as gout is less likely now ,you need to look at other causes like osteoarthritis which is common degenerative joint disease and also soft tissue infection of the finger by bacteria.I would like to know further more history and want to see pictures of the involved finger before I can come to conclusion. Before that for symptomatic relief and to prevent infection,I would advise you to take third generation cephalosporin orally as antibiotics and take analgesics like Etoricoxib which has no gastric side effects unlike other NSAIDS,also apply analgesic gel topically.Put ice on the painful area for relief . Regarding your previous query, yes tart cherry juice moderation is due to its gastric side effects not blood thinning. If you need further assistance please contact me.Im happy to help you.
Thanks.
