Brief Answer:
Please refer to the detailed answer below

Detailed Answer:
Hello Dear

After going through the medical details provided by you I can understand your concern for your mothers health and I would like to tell you that transcatheter aortic valve replacement in experienced hands carries a good prognosis.

As the age of your mother is 70 so making her undergo another open heart surgery will carry more complication risk as compared to TAVR. However TAVR is also a complicated procedure but is very less invasive and only very few patients meets the criteria to undergo this.

Probably the aortic regurgitation is severe and your mother is symptomatic that’s why your treating doctor has advised another surgery.

If she is a suitable candidate for TAVR as per your cardiologist and you can handle the finances then it’s a better option.

Kind Regards

Hello again

Detailed Answer:
Yes it can be done without open heart but require extreme expertise for that. I suggest you to discuss this with cardiac surgeon and cardiologist both.

Her TEE report shows a doubt of having a vegetation (infective endocarditis) on the valve leaflet which is the probable cause of her valve dysfunction. Does she had any persistent fever anytime after her valve replacement?

Detailed Answer:
So basically this valve failure happened as a result of infection which hopefully won’t happen again in future.

Yes that’s surprising to me also because both were TEE done during a short time frame.

No it’s not advisable to operate defibrillator without training or medical supervision.

Here’s the link Mr XXXXXXX

The problem with mechanical valves for elderly patients is that they have to be on anticoagulants for the rest of their lives. Now with anticoagulant there is significant risk of major bleed anywhere in the body which itself can cause threat to life. For this reason usually mechanical valves are not preferred in senile age group.

Hello again

Detailed Answer:
Yes, some surgeons Prefer leaving a window of pericardium open after surgery in some patients so as to avoid any postoperative fluid collection.

Now pericardium does act as a cushion and absorb vibrations coming from heart and when a window is left there heart sounds become more obvious.

If you are planning to go for open heart surgery you can have a discussion with the surgeon about this as in my knowledge there is no way to block those sounds.

Detailed Answer:
Am sorry but this is a quite vague sort of information to make any diagnosis out of this. If it was a external defibrillator it cannot be just a wire.

Also without having a look at the ecg it’s impossible to say what was the event because there are several types of tachycardia causing arrhythmias. So I don’t want to furnish you with any information which is guess based.

Detailed Answer:
Mortality rate varies as per the clinical situation of the patient for redo open heart surgery.

For clinical stable and relatively a symptomatic patients risk varies around 4-5 % and for symptomatic patients it can be as high as 18-34%.

TAVR is a better choice if patient fits in the criteria for that as risk is significantly low due to its less invasive nature.

Kind Regards

Preferably the model that provides the largest valve effective orifice area in relation to the patient’s annulus size is selected. Which valve has what area that a cardiac surgeon can only guide.

They are giving a time of 1-2 months probably because they want to stabilise the patient and optimise the medication.

Hello

Detailed Answer:
You need to read developmental embryology to understand why it’s dual and triple leaflet. It’s not simple and can not be explained here.

As I said earlier 2 leaflet or 3 leaflet the choice depends upon the EOA (effective orifice area) indexes to every single patient specifically and is estimated and decided by the team including echocardiographers and surgeons.

Hello

Detailed Answer:
Yes it’s a recent development in aortic prosthesis and it has better compliance because it has more passing area for blood therefore low pressure across the valve causing less wear and tear and hence longer life span.

Trileaflet will be a better choice.

I think you are misinterpreting, TAVR is basically just a technique of valve replacement and not any sort of valve itself.

There are two types of valve options in TAVR and both of them are now trileaflet and bio valves.

TAVR in comparison to mechinal valve has less lifespan because it’s tissue material and degenerate with time.

It’s all because of the material used. In TAVR the valve has to be very delicate and flexible as it’s taken inside the heart in a squeezed shape.
