default
Posted on Thu, 17 Oct 2019
Question: My psych prescribed seroquel and klonopin for my anxiety induced insomnia. Worked well for the last four years. Suddenly stopped working. I tried otc fast dissolve melatonin 2.5 mg with my meds and it helped put me to sleep but gave me a massive hangover. Would just like some feedback second opinion on what to try next. Is the regular melatonin better for me to try? I take 150 mg seroquel and 1.5 mg klonopin. Psych wants to bump up dose to 2 mg klonopin and 300 mg seroquel. I actually tried this last night and it did not work. I took right before bed and of course I wasn’t tired at all.

Consultation

Detailed Answer:
Hello,
Thanks for using Healthcaremagic.

I read your query and understand your concerns.

Although it is never possible to find out exact next option on basis of limited information but from the available description I feel increasing the dose of quetiapine by 25 mg every three days, till suitable dose found should be the next best option in current scenario. I mean to say that rather than using 300 mg directly go slowly to 175 mg of quetiapine, 200 mg of queatiapine and so on gradually to find out the next possible suitable dose.

My recommendation is based on the fact that combination of seroquel and Klonopin worked well for you upto four years and sudden disappearance of effect indicates that there is need to change dose of medication in gradual manner rather than abrupt manner to find out the next possible effective dose.

It is important to state that it is common to have loss of effect when benzodiazepines are the part of treatment (read klonopin) and increasing the dose of Klonopin never solves the problem in long term although it could be an effective short term solution.

I hope this helps you.
Feel free to write back to me if you have more questions.
Thanks and regards.
