Dear doctor,
My son ,20 ,has a following problem since last week
1.Frequent urination .more sensation in the night than day.
2.ringing sensation in ear particularly when he undergoes sleep.
3.He is right now undergoing diet plan in which last week he consumed D protein ( a powder available in market.)about 2 tablespoon a day for 3 days.and just after that this problem started.
4.local doctor prescribed urispass -3 times a day and Cefixime tablet twice a day.but not helping much invreducing symptoms.
5.He tested his glucose leval on glucometer which was normal.
We read on internet that excess vitamin D
causes these problem.
He does not have cough,cold ,fever and no fatigue.
extremely worried .pl consult asap.

I have noted the detailed summary of the problems your son is facing.
Since the problem started after taking D Protein powder, you should stop that immediately.
For ringing sensation in the ears, he can take Tablet. Vertin 16 twice a day for 5 days.
Frequent urination without fever or burning or pain while passing urine is more likely to be due to the current weather.
You can ask your son to reduce water intake. It is okay to drink about 2 liters per day.
Since Cefixime has already been started, please complete the course of 5 days.
If I was treating him, I would started appropriate antibiotics only if there was evidence of infection in the urine.
You can confirm if there is infection by getting Urine Routine and urine culture test done, CBC will also be useful.

The urine culture is a more specific test for urine infection. If there is infection, then it will show which bacteria is responsible for the infection. It will also tell us which medicine will work on that bacteria.
For urinary tract infections, I prefer Ciprofloxacin or nitrofurantoin as an antibiotic.
So, if the infection is still present, You can ask your local doctor to prescribe ciprofloxacin 500 milligram twice a day for 5 days after food. (That is if the bacteria is susceptible to ciprofloxacin).
If the bacteria is resistent to ciprofloxacin, then whichever medicine it is susceptible to, only that should be prescribed.
Don't worry, it can be cured with proper antibiotic course.

Thank you for sending in the reports, I have seen them.
Except slightly more than normal pus cells in the urine, every other parameter is absolutely normal.
CBC is perfectly fine, so is blood sugar level, thyroid hormone level.
To be honest, I wouldn't start cefixime in these kinds of patients. Nitrofurantoin (Tab Niftas 50 mg 4 times a day after food) is what I would prescribe and add a specific antibiotic like ciprofloxacin 500 mg twice a day for 5 days after confirming the infection on urine culture test.
Let him continue drinking about 2 to 3 liters of water per day.
Other than that, local hygeine is essential.
There is nothing worrisome in these reports.
