Brief Answer:
Osteoarthritis possible, no HIV test needed.
Detailed Answer:
Hello dear, welcome Ask a doctor's service.
I read your query and here is my advice.
I see the photo you attached and your hand is having really protruding knuckles or knoby bone deformities. The joints of the small finger seem more deformed.
I believe this can be early signs of osteoarthritis.
The throbbing sensation possible is as a result of intense work you may have done this morning and because of the inflammation too.
I believe you should consult a rheumatologist about possible signs of osteoarthritis and doing a x-ray of hand.
Corticosteroids are one of the drug groups used to treat osteoarthritis.
About the incident you mentioned with that piece of wood I think there is no HIV risk and no need to do HIV test.
The HIV virus is fragile and can't resist longer outside the body.
So no HIV test needed.
Hope I have answered the question.
Let me know if I can assist you further.
