Question: My sister is a very heavy drinker. Lately she has lost all of her weight, she is skin and bones, literally. She is afraid to eat because she has severe hemoroids and takes Milk of Magnesia, about a bottle a day, to keep it from hurting when she poops. Her face is real puffy. It is the largest part of her body. Is this very serious? She refuses to let them test her liver. She just keeps drinking. She did pass out at work the other day for no reason.

Brief Answer:
Puffy face probably due to malnutiriton

Detailed Answer:
Hello,

The swollen face is most probably due to malnutrition. Her body proteins seem to be getting low. Since she passed out, it also can mean electrolyte imbalance.
Since she has been drinking heavily, we also need to test function of organs like liver and kidney. These can also get damaged and result in puffiness of face and body.

Kindly get her basic blood tests done like liver function test, kidney function test, fasting blood sugar and serum electrolytes. Also ultrasound whole abdomen is needed.

Yes its serious and she needs help of a surgeon/doctor preferably at a hospital.
Because of her drinking, her judgement is probably impaired. Her caregiver can legally take medical decisions on her behalf. Check with nearest hospital about it.

Hope this helps. Let me know if I can assist you further.
