Brief Answer:
possible internal bleed. ER visit recommended.

Detailed Answer:
Hello and welcome to Ask A Doctor services.
I have read your query and here is my advise.

* As your son is recovering from stroke, currently on blood thinner with
blood at the tip of penis indicate internal bleed most likely.

* If there are no associated symptoms as burning urination or dysuria, there
are all chances of alteration in coagulation profile in terms of bleeding or
clotting time most likely.

* It is strongly recommended to withheld further doses of any blood thinner
till consultant advise.

I would recommend an ER visit for best evaluation right now and prevent any further complications which may follow by waiting till tomorrow morning.

Wish him best recovery and health.
Do feel free to discuss further at any point of time.

Regards.
