Question: My son has been suffering from quite severe lower back pain for the last 24 hours. He feels this is being caused by a urine or kidney infection. He feels feverish, but does not have nausea or vomiting. He works outdoors all day. Could the cold be making it worse. His urine is quite dark in colour . Does alcohol make this kind of infection worse. He has recently been drinking more than usual.

I carefully read your query.

Kidney infection generally causes pain in the back that is closer to the waist area, close to lower ribs. It is a pain that is much worse than any direct hit-injury. He also would get urinary symptoms like burning and frequency in urination.

Dark urine can happen with urinary infection but also if he has not been hydrating well. Furthermore, alcohol itself can be dehydrating and cause such.

If the pain is in the midline or is on one side but closer to the lower back, above the hip area (not close to the rib area) then very likely this is muscular pain, especially if the pain is worse with certain movements and he has no urinary symptoms.

To conclude I would say:
- it is advisable to drink plenty of fluids (at least 3Ltr a day)
- he can take Acetaminophen (Tylenol) to help with the pain
- if the pain is on one side, close to the rib area and he has any burning sensation in the urine or frequency, he will need to have a urinalysis to rule out urinary infection. In young males with urinary symptoms, if sexually active, the doctor might ask for more tests to rule out the risk for sexually transmitted infections as well.

- it is good to check the temperature when he feels like he has a temperature. If his temperature is 38°C and above he is feverish and infection is likely.

- As per the clinical findings and the results of the investigations the doctor will be able to decide on proper treatment.
