The main thing to watch out for are signs of dehydration if he is not able to drink enough fluids, which you may notice if there is decreased urine output, or if he cries but there's no tears.
Unfortunately, there's no cure for the virus that causes this disease, so you can only treat the pain and fever with Tylenol (paracetamol) and Ibuprofen. You can use both at the same time, or switch between them, using doses as directed for his weight. Hopefully this will help him to be more comfortable while you wait for the body to clear this illness.

If he is still having fevers after 4 days, I would take him back to the doctor to be examined again and make sure there is not something else going on or a complication of the Hand foot and mouth disease. Usually by 4 days the fever has resolved, so it would not be normal for him to still have fevers after this point.
If it is still day 4 of the illness you could still watch him at home as long as he is drinking fluids well, responds to you normally (he may feel tired, but it should not be difficult to arouse him), and is not vomiting. But if he continues to have fever after day 4, you should bring him to a doctor.
