Question: My son is 19 and is constantly cold. His hands and lower legs and feet are literally freezing cold to the touch.

However, for the vast majority (like, 99%), this is just a case of having a normal body, just having colder hands and feet than most people, just as some people are very tall or very short, but it doesn't mean anything is wrong with them.

Rarely, cold hands and feet can be a sign of an underlying disease, such as peripheral artery disease, rheumatologic conditions, or thyroid problems, but at your son's age, these would be very unlikely. If you son doesn't have any other problems, especially any skin problems, I wouldn't consider it worth checking for these. Raynaud's disease can also cause cold fingers and toes, but this is usually temporary and associated with color changes in the skin.

Your son can deal with his cold hands and feet by keeping them warm, and avoiding caffeine and nicotine, which may make the blood vessels to the hands and feet constrict and make the coldness worse.
