My Son Is 3 Months And 4 Days Weighs About

Don't worry, continue feeds, probiotics will help
Detailed Answer:
Hi
Thanks for asking on this platform
I have gone through the query and do understand your concern.
I won't get overly worried if baby is otherwise active, there is no abdominal distension or vomiting, taking feeds well and passing urine.
Change in frequency and consistency if it is not significant enough to cause other problem is normal in infant on breast feed and formula feed. Passing motion after each feed is most likely due to hyperactive gastrocolic reflex which resolves itself within 3-4 days in most of the cases.
Sometimes there is transient alteration in gut flora which may cause change in bowel pattern and smelly stool. Giving probiotics containing lactobacillus or saccharomyces(Econorm or Enterogermina) for 4-5 days helps in such cases.
Watch for frequency of urine as it gives an idea about the hydration status of baby.
Continue giving feeds as usual and observe for any significant issues like too frequent or too watery stool, blood mixed stool, perianal rashes, vomiting or abdominal distension and if you notice any please consult with your doctor.
I hope this helps.
Please let me know if you have any more questions.
Regards

Get stool test done for reducing sugar
Detailed Answer:
Hi
Thanks for follow up
I have seen the image and there is perianal redness.
Perianal rashes suggest increased lactose content in stool. pH of high lactose containing stool is low and this acidic nature of stool causes skin redness.
This is due to transient lactose intolerance and is common with increased frequency of stool.
Here I would suggest you to get lactose(reducing sugar) level of stool. Changing formula for 5-7 days to low lactose or lactose free one will help.
Most of the time this lactose intolerance is transient and you can restart the old formula after 5-7 days once the stool pattern gets normal.
Continue giving probiotics.
I shall be glad to help further if you need any.
Regards
