My Uncle Has A Severe Deep Vein Thrombosis (DVT). It

Question: My uncle has a severe deep vein thrombosis (DVT). It started in 1996, when he was 26 years old. He broke his leg, doctors applied cast, after remove of the case, the leg started to inflate (become bigger and bigger). During year of 1996, doctors said that inflation of the leg is normal and mention it will heal on its own. In 1997, he started experiencing ulcers on his calf it was small dot but now as you can see on the pictures it big. At the present time my uncle is 49 years old.
Right leg DVT/ulcer
His height is: 193 cm or 6”3
Weight: 142 kg or 313 pounds
The length of the ulcers wound is 2.7 inch by width 1.9 inches, depth of the wounds is approximately 1-2mm
-     Feeling fatigue/restless when he walks to much
-     Blood pressure is 130/80
-     Urine sugar levels are normal, blood sugar level 7.1, he will repeat the test again for blood
-     DVT start at age 23, ulcers appeared at age 26
-     Heart history is normal
-     Sleeping pattern are normal
-     Eating pattern are normal
-     Used to drink alcohol but stopped in 2010.
-     Breathing is normal
For 20 years, the treatment was limited, he just used regular water and hydrogen peroxide to clean it. He did go to the doctors, but they didn’t help much. For past 6 years, if the wound leaks, he uses hydrogen peroxide, cleans of the edges of the wound, cleans the middle and then applies either sulfanilamide or his urine. Sometimes it works sometimes its not.
-     When his touching his calf it hurts, if he grabs the calf and applies pressure it hurts 10/10. If someone punches the calf lightly its 10/10 pain. If he grabs the calf and applies pressure the wound doesn’t leak.
-     Wound leaks on certain days when he walks. He could walk 30 mins and it starts to leak, it can be half a day and it starts to leak, or full day and it starts to leak. Leakage is various on the day, some days its little and some are a lot. (when it leaks, the color is red, seems like blood).
-     If he lifts something heavy the wound starts to leak
He started smoking in 1985-1990 casually, one cigarette per day. From year of 1991 to present, he smokes one pack per day.

Myself I am his Nephew, living in Canada. He got accept to visit Canada. He has a flight book May 6th to arrive to Canada. I want to help my mother to fix his brother injury/wound. What recommendation can you give me for this particular situation. is their certain antibiotic (pills) he can take, is their a surgery that can be done to fix this problem. what is the full treatment process he need to be in order to heal the entire leg?

What are some blood thinners can you recommend taking that can be bought of the shelf that are not prescribed by the doctor?

Based on his condition as you seen the photos, how long can the treatment take base on following the steps of taking apixaban twice daily for 3 months orally, ciprofloxacin 500 mg and Mupirocin cream?

Because his 313 pounds plus smokes cigarettes a pack per day, if he losses weight plus eliminates from smoking a pack cigarettes to smoking to low as 1-2 cigarettes per day will it help to heal the ulcer wound/DVT much faster way?
