Question: My wife has a very swollen and tight upper left shoulder. Concentrated in that area. No other symptoms.

I read your query and here is my advice.

I think she is suffering an injury of rotator cuff or tendinitis of one of tendons attached to shoulder.
I can can be caused probably by a sudden movement, or repeated movement and overusing the arm. The main symptoms are pain in shoulder, swelling and inability to use arm as usual.
She can do some tips such:
Use ice compresses over the shoulder.
Put the arm at rest and avoid movements which cause pain.
Use ibuprofen or naproxen for pain.
Also physical therapy may be used successfully.
