Question: My wife has given birth 11 months back and now she is facing pile problem.
she is facing piles problem from almost 7 months.
she is getting pain very often.
what are the options or medicines to cure piles problem?

Is bleeding minimal?
[ANSWER]: No Bleeding at all.

Any swelling all over the anal area?
[ANSWER]: No swelling

Or 2 skin tags - one in front and the other behind and no other swelling?
[ANSWER]: No swelling but We could see one skin tag

Hard stools?
[ANSWER]: Yes. pain starts only after hard stool

Any mucus in stool?
[ANSWER]: NO

Any abdominal complaints?

Did she consult any General Surgeon for an actual examination/proctoscopy to know the exact problem?

Thanks for your detailed feedback.
This is classical history of a condition called fissure-in-ano.
The cause is obviously the hard stool rubbing on the skin at anus causing the cut; the area being very sensitive causes more pain even for small cut. Super-added infected due to bacterial contents of stool may cause additional mild to moderate infection and may need ''prescription based'' antibiotic safe for the baby too.
For keeping the stools soft enough different stool softeners and laxatives can be tried. Lactulose action is mild hence needs to try others like Fabolite powder OR Cremaffin. Doses as per your Doctor's advise and the dose to be adjusted as per the results (casing softness of stool). Every persons doses are different.
Next thing is Sitz Bath:
Sitting in a squatting position in a large tub. The water should be warm and temperature to be checked by back of hand before sitting to see that the water is not too hot. Needs to be done before passing stool, after passing stool and in the evening if possible.
Foods and beverages to be adjusted in such a way that those which cause constipation or hardness of stool should be stopped.
Above mentioned things help most of the patients.
Additional treatment if needed may be in the following ways:
Application of Xylocaine jelly at the fissure area before and after passing stool and when the pain is persistent.
Painkiller with anti-inflammatory properties like Ibuprofen if it suits her.
Prescription based antibiotic as mentioned above.

Additional if Multivitamins with mineral and trace elements. Your Medical Shop owner/Pharmacist can tell you the best available.
Nothing different to be done at the moment.
Once the pain is settled you can go for proctoscopy to see for any piles.
