Brief Answer:
If USG shows inflammed appendix it is best to get surgery done

Detailed Answer:
Hello,

Welcome to Ask A Doctor Service.

In your query you have mentioned Appendix of 4 mm, there is no mention of inflammation of the appendix and any other finding so it is difficult to give you any useful guidelines.

Can you please upload the full report and if blood tests are done, upload them as well, I will review all the reports and give you further advise.

If the appendix is swollen and inflammed, then it needs to be removed by surgery, as there are chances of the appendix bursting open and pus spreading all over.

If the appendix is in fact inflammed, please get your wife admitted in a hospital and get the surgery done.

I hope that answers your query.

Best wishes
