Question: My wife was on PPIs for 4 months, and while the exact cause it not clear, for the past one month she had complained of not being able to sleep at all. We went to a neurologist who gave Clonazepam , .5 mg for 10 days and .25 mg for the next 10 days along with melatonin. She was able to sleep after this. After 11 days, she noticed swelling around the ankles. Searching online, this is listed as a rare side effect . Should the .25 mg dose be stopped for a few days, or should we ignore the issue ? Abrupt stopping too could cause problems is what is mentioned for this drug

I have noted your wife's clinical and medication details. Based on this, I feel she needs to be investigated to look for causes of swelling around the ankles. Blood tests needed would include- creatinine to rule out kidney problem, liver function tests and albumin (as low albumin can also cause ankle swelling). If all these come normal, only then, we could consider clonazepam as the cause, as swelling around ankles is not common after clonazepam use.

If clonazepam needs to be stopped, it should be gradually tapered and stopped. After 0.25 mg daily for 10 days, it can be given once in 2 days for 5 more doses, and then stopped.
