Myfeetand Ankles Have Been Severely Swollen And Painful For Several

Question: Myfeetand ankles have been severely swollen and painful for several days. What can I do to reduce swelling besides elevation which isn't working. Also yesterday I cut my leg and after the bleeding stopped clear fluid drained out for hours.

Firstly,you should not give a cut on your feet to drain the fluid as this would expose you to infection.

As the swelling is bilateral and you don't have any history of trauma hence it seems to be a result of some systematic conditions (and not a local cause).

Let me know if you are taking any medications like steroids ,Pain killers NSAIDs, BP medicines(specially amlodipine), diabetes medicines called thiazolidinediones or estrogen for post menopausal state.
It may be secondary to such medication.

It is also common in Hypothyroidism so have you procured thyroid levels in near past?

This is common in high blood pressure and congestive heart failure.

In Congestive heart failure pumping capacity of heart is compromised causing peripheral edema.In swelling due to heart failure,skin would remain a dimple after compression (Pitting edema)

Fluid may accumulate in feet as a result of hypofunctioning liver where plasma proteins are not formed in optimum amount.

When there is kidney disease, extra fluid and sodium circulation are not filtered in urine and may cause feet edema and periorbital edema.fluid accumulation and edema

 If there is chronic venous insufficiency, the one-way valves in long veins of leg are weakened or damaged which allows blood pooling and cause feet swelling.

Swelling in one feet may be a DVT which is an emergency but is usually unilateral.

Visit your clinician and get your liver function and renal function tests and 2 D echo done.If need arises,you may need a lower limb Doppler.

To such patients visiting my clinic,I usually start mild diuretics (fluid pill) like Toresamide in the dose of 10 mgm in the morning.That removes extra fluids and water from body.
Protein supplements help to mantain oncotic pressure and prevent fluid leak.

Mantain ideal body weight,avoid prolonged sitting and standing,continue elevation,have a salt restricted diet,avoid alcohol and smoking, do compression with stockings or crepe bandage.
