Question: On the 28th December I had a very light 3 day period. I always have a period every 27 to 30 days. I got this very light bleeding on the day my period was expected. I only needed one towel each day. I usually have 5 day periods but this was completely different and very light. I have done a pregnancy test as I am trying to conceive. Do you see I could still be pregnant or is this ruled out. I am 44 and have only just tried to start conceiving. Thanks

If you have done the pregnancy test soon after the period, you are not likely to get a positive result even if you have conceived because of the low hormone levels in the urine. Instead, you can go for the blood test which is highly sensitive.

Alternatively, you can repeat the urine HPT with a fresh early morning sample now, i.e., a week after the period. This can be supported by a trans-vaginal/pelvic sonogram for further clarification.

Scanty menstruation at the time of the expected period can be seen with an implantation bleed. So, I suggest clarification of the conception status through testing.

I would also ask you to go for clinical assistance if you fail to conceive in 3-6 months in view of your age where the ovarian reserve is limited and hence one cannot afford to waste precious ova.

I have been to my GP he is going to send me for an ultrasound and is doing a whole batch of bloods. I am worried this is the start of menopause with the odd period I had or odd light bleeding. What can be the problem other than menopause?

Please do not worry. Let the investigation be done. Your condition is more likely to be a transient hormone imbalance if pregnancy is excluded. Early menopause is suspected with positive family history and conditions that could precipitate like a history of some pelvic surgery etc. This is very unlikely.

Scanty menstruation does not rule out ovulation. However, as I mentioned earlier, it can be seen more commonly in hormone disruption when the endometrial preparation is lesser than normal or when there is an implantation bleeding. In the latter case, you should have a positive pregnancy test by now. A trans vaginal sonogram is a better way of clarification than repeated urine tests in the current scenario. Further management can be planned accordingly.
