
Over The Past Few Weeks, I Have Been Having Medical

Question: Over the past few weeks, i have been having medical problems.
The problem started off with pain in my left testicle, between my testes and anus ( dull pain when i press the left side ) and lower abdominal pain.
I visited the Urologist who first diagnosed me with Prostitis and told me its infection of your prostate gland and would subside over few weeks with medication he gave me ( herbal ) . The pain continued so i went to visit him again and he diagnosed me with varicoccele grade three in my left testes with 3mm dilation. I am also having some symptoms of pin like pains around my middle abdomen and anus, hard time passing feces and when i do, its lose feces. The pain in my testes radiates to my inner thigh as well. I want your opinion

The ultrasound report says left and right varicocele with more in left side.
When urinating there is sharp pain.
No swelling in scrotum or left testes. The sizes are normal
Pain inside the left scrotum feels more tender than right side.
No rectal examination done.
To also mention, i have lumbar spine problems, herniated and dehydrated L4 and L5 with S1

The details about pain you have mentioned makes me think of hte following:
Pain on the left side of scrotum and upper thigh can be due to nerve pinching syndrome that supplies this area. Most probably Genitofemoral and Ilioinguinal nerves. The pinching can be from its origin in spine or in its course to the skin; enlarging inguinal hernia also can cause stretching of these nerves and cause similar pain.
Or there can be conditions like Epididymo-orchitis which can cause similar pain and tenderness with referred pain to mid and lower abdomen near center and so on.
Varicocele causes dull pain only which is relieved on lying down, the visible swelling increases in standing position and on straining.
Since you must have mentioned pain towards anus and difficulty in passing soft stool, your Urologist must have though about prostatitis.
The best option to know about prostate is per-rectal digital palpation supported by per-rectal or abdominal Ultrasound.
Lumbar spine problem has complicated the diagnosis further.
Hence I would advise you the following to be discussed with your Doctors/ Specialists.
Proper clinical evaluation, an actual examination of abdomen, scrotum and per-rectal examination.
Urine: Routine, microscopy, culture and sensitivity.
Tests of blood for CBC, Sugar, Kidney functions.
Ultrasonography of abdomen, particularly for genito-urinary system and of Scrotum
Any other tests as needed.
All these will give proper diagnosis which helps in deciding proper management.
