PEP Within 10 Hours After The First Exposure Can Be Useful

default
Posted on Tue, 13 Feb 2024
Question: 1. Oct 25th my condom slipped when I pulled out since I panicked, concerned my Doctor started me on PEP dolutegravir, lamivudine and tenofovir disoproxil which was started in time (10 hours)

2. I have been on PEP for two good weeks and had a very similar incident, where the condom slipped again. I don’t know the person’s status is unknown. NOV 8, this is quite a bizarre instance of occurring twice. I think I switched to a new condom brand.

I have clarifications here:

. I am still taking PEP and have 15 more days of dosage left which will end on Nov 28th. So given that I am already on. The meds and the risk exposure of the issue should I just complete rest of my course and extend the PEP additional 2 weeks? My family doctor told its no need to extend that course...but remaining 15 days you will strictly followed.is it true doctor?

Brief Answer:
Not necessary to extend the course of PEP.

Detailed Answer:
Hi,
Thanks for posting your query.
As you had initiated PEP within 10 hours after the first exposure and first slip, you are absolutely safe from HIV infection and not from the risk of other STIs. Complete the course correctly. You have to follow the other protocols of PEP like repeated testing for HIV (on day 0, 1 month, 3 months and 6th months) and follow ups till 6 months and after seen the HIV nonreactive test report, you can resume normal sexual life, till then abstain from sex or practice protected sex correctly.
As you are already on PEP, it will act as a PrEP (preexposure prophylaxis ) for the second exposure and the condom slip. So not to worry. Your PEP will take care. Not necessary to extend the course.

Brief Answer:
PrEP is not indicated for you

Detailed Answer:
Hi,
Welcome back.
PrEP is meant for those who at risk of getting HIV infection like uninfected partners of HIV positive individual (discordant partner), Men sex with Men (Multiple partners and Commercial sex workers and IV drug abusers and not for casual contacts. Those who are at risk for HIV infection should take PrEP as long as they are sexually active on daily basis. Occasional casual contacts can have a protected sex. If you use PrEP frequently and intermittently, there is every chance for developing resistance by the HIV to those drugs. In case if you got infected in future in some odd occasion, you will loose good drugs in your armaments package.
