Question: Paediatric Request

This request is for our daughter "Shraya Pradhan" who is 14 months old. She had a fever a couple of weeks back which got cured with babygesic drops in a couple of days.

After that, we are noticing she is not eating well. Nowadays she tends to show vomiting symptoms on seeing the food, and with every bite as well. But she is not vomiting. Please assist.

2. Is she otherwise active and playful?

3. Is she taking enough breast feeds?

Probably she had a viral illness couple of weeks back. It is common for babies to have some anorexia(poor appetite) following a viral illness. In most of the cases, this will revert to normal in 7-10 days. Rarely, as in mononucleosis, this may take 4-6 weeks to settle. If she is otherwise active and playful, we need not worry much

At 12 months her weight was 7.5 kg
Her current weight is 7.7 kg.

2. Is she otherwise active and playful?
Yes she is very playful and active

3. Is she taking enough breast feeds?
Yes

4. You mentioned iron and vitamin D supplements- when this was started, and whether any blood test was done before that?
This has been recommended by the doctor we consult regularly. Ultra d3 since birth, and iron from last 6 months (before that she was taking zincovit). No blood test was done.

5. Exactly when was that fever, and were there any associated symptoms like cold/cough/loose stools/vomiting/rash along with fever then?
Fever 1 - 30 March - Fever 99-101 Celcius. Consulted a doctor. Normal Urine test. Medication - Only Babygesic for 3 days
Fever 2 - 16 Apr - Fever 99-100 Celcius. No consultation. Medication - Only Babygesic for 2 days

After fever 2, she has lost appetite for food.

No serious symptoms as such. Once in a while, she got a little runny nose, for which we gave saline drops in the nose.

Thank you for the details given; it was presented very nicely

So as I mentioned before, any febrile illness will be followed by anorexia/poor appetite for few weeks. Since she had 2 episodes of viral fever in last one month, let us wait another 3-4 weeks for her appetite to return. Until then no worries at all.

Meanwhile, you increase the frequency of feeds instead of force feeding. Give only the amount that she accepts, and repeat this every 3-4 hours. Adding oil/ghee/sugar to food items will increase the energy density and promote weight gain. Also, give rice water or undiluted milk to drink, instead of plain water. This also will improve weight gain. Include protein rich items(dairy/meet/fish/egg) and smashed vegetables&fruits in diet.

Avoid giving much tea/coffee/chocolates- these may suppress appetite.

Regarding supplements, we need not continue Iron beyond 6 moths unless there is documented anemia(Hemoglobin level <11g/dL). Also, we can stop Vitamin D after 1 year of age.

In short, be relaxed and wait for 1 month; follow the above diet measures and get back to me after 1 month with her weight. Let us see. Hopefully she will improve by then
