Looking into your problem I would say that Hiatus hernia may lead to these symptoms due to gastric reflux causing pain and burning sensation along with palpitations due to irritation of vagus nerve but it is very unlikely to have raised Blood pressure as you are mentioning in your history .
Panic attacks can lead to sympathetic nervous system stimulation leading to these symptoms in which you can experience sudden elevations in BP due to anxiety . Mouth pain can be due to gastric reflux as you were lying down during sleep for prolonged periods and in hiatus hernia swallowed food balls up and causes irritation of lower esophagus, a movement that is facilitated by lying down position .
I advise you to XXXXXXX a gastro surgeon for this problem and he may prescribe some antacids and drugs decreasing the reflux which may solve your problem .
