Hello,
Thanks for posting your query.
The pain in your shoulder and upper chest could be due to reasons such as:
•     Periarthritis
•     Frozen shoulder
•     Inflammation of the bursa of shoulder.
•     Cervical spine disc prolapse.
•     Heart disease
As I believe that more information helps a physician arrive at an accurate diagnosis, I would request you to answer the following questions:
a.     How long have you had the pain?
b.     Does the pain increase on movement of the shoulder joint?
c.     Does the pain increase with exertion/ breathing in/ cough?
d.     Do you have any history of hypertension (increased blood pressure) and diabetes?
e.     Do you have any family history of such a problem?

I would suggest that the following investigations be done to come to an accurate diagnosis:
•     Electrocardiogram
•     Chest Xray
•     Xray of the shoulder joint
It is important that heart disease be ruled out first. I would suggest that you consult your prescribe appropriate therapy.
Awaiting your reply.
Regards
