Question: Please help me understand these results

MRI Results
I have had lower back pain for many years. Recently the pain has moved to both of my hips. I also do not have reflexes in my knees when tested by my doctor.

L3-L4 There is broad-based posterior disc osteophyte complex which effaces the ventral aspect of the thecal sac
L4-L5 There is broad-based posterior disc osteophyte complex which effaces the ventral aspect of the thecal sac
L5-S1 There is small central disc protrusion without central canal or neuroforaminal stenosis
the nerve roots of the cauda equina layer dependently within the thecal sac

Thanks for posting your query.
I have seen the report mentioned. You have spondylottic changes in the spine. These are age related degenerative changes and cause an impression or compression of the spinal cord or nerve roots. IN your case there is just a mild impression on the thecal sac. There is no nerve root compression or irritation. Regular physiotherapy and back strengthening exercises can help in maintaining the flexibility of spine and will help in preventing further deterioration.
The pain in back and hips can be related to a muscle spasm. You can do a hot compress and apply a local analgesic gel for relief.
