Question: Psychiatrist only.
I have severe ocd and if I think something that I don't want to think, I wanted to wash hands. Why? Male 38.

I can understand your problem. Obsessions are nothing but intrusive, repugnant, unwanted ideas, images or impulses that dominate in mind and result in stress. These are totally purposeless and individuals have insight that these are purposeless still these persist in mind and result in stress. In order to relieve stress individuals result in acting according to the thoughts which result in compulsions. In your case hand washing which is repetitive is result of obsessive thoughts.

To control this behaviour detailed evaluation is needed and treatment is must. Treatment comprises of both medicines and behaviour therapy. Medicines like SSRI which are anti anxiety drugs are mainstay of treatment while thought stopping, exposure and response prevention, relaxation exercises are mainly part of behaviour therapy.

You should provide me details about the thoughts you have and we can proceed further.

I get stressed about false memories. I keep on thinking like in pure ocd. I have to think a thought to cancel an intrusive thought. It's like a non stop loop where I m doing a ritual of thinking to cancel unwanted thought. Pure obsessional OCD.

I m male.38. I m t2 diabetic. I m on luvox 100 mg, antacand and metformin. I have tried cbt and erp also and antipsychotics also but to no use.

I can understand the problem. Even if it is obsessional only even then this should be controlled totally. The dose of Luvox you’re taking is 100 mg and this can be increase in my opinion if the symptoms are persisting. In OCD doses up to 300 mg per day can be used to see desired benefits. If with optimal Luvox dose benefits are not seen then drugs like Clomipramine can be used as augmentation treatment. You should talk with your doctor for this. This should definitely help you.

Hello again

What I do in my OPD is to advise patients to start with low dose like 50 mg and to increase it to 75mg in a couple of weeks. The dose can be increased to 150 mg with Fluvoxamine. Even higher doses can be used. Most individuals see increased sedation, constipation with medicine. So it’s better to take the drug in evening. You can talk with your doctor for this.
