Hi friend,
Welcome to Health Care Magic
Sorry to see the situation of your grandfather.
It is difficult - not impossible to come around...
In olden days, pneumonia was called 'old man's friend' - because it was a terminal event due to lack of treatment.
Nowadays, with advances in antibiotics, respiratory support and so on, there is hope (though not bright) - in spite of his age and heart attack.
Take care
Wishing speedy recovery
God bless
Good luck
