Brief Answer:
Fungal infection of skin and nails

Detailed Answer:
Hello and welcome to ask a doctor service. I have viewed the image and gone through the query.

This seems like a fungal infection of the skin as well as toes(onychomycosis). I suggest you to use a topical antifungal e.g either ketoconazole 2% cream or miconazole nitrate 2% gel, twice daily. In addition you may also take an oral antihistamine e.g cetrizine 10mg tablet once a day for symptomatic relief from itching. I would also suggest an oral antifungal e.g Terbinafine 250mg once a day for a few weeks.

Hope I have answered your query. Let me know if you need any more assistance.
