Your concern as a son for your father is appreciable.There are 3 main issues that you have mention in your query.
1.high blood pressure:his blood pressure is quite high,though he is taking medicines,the blood pressure is not under control,which needs to be taken care at the earliest.
2.Piles:At his age,there are chances that he might be having constipation and habit of straining at stool,this can lead to development of piles.
3.Pain in leg:There could be various reasons for pain,first of all pain due to some deficiency of vitamins like B12 and calcium,secondly there are chances of some form of clotting his veins of leg,may due to fat deposition or may be due to some circulation problems,that probably the reason for high ESR.
Please do the following.
1.Take him to a physician/cardiologist and get him examined properly.Ideally he should be advised proper blood investigation like tests for kidney function,blood sugar,EKG,2 D echo,ultrasound for kidneys and Doppler study of both limb.The list could be more or less as per the examination.He will adjust the dose of drugs or add some new one,including the drug for improving circulation as per need.
2.After his blood pressure comes under control he need to visit a surgeon to see degree of piles,whether they require surgery or not.It is possible that leg pain may be due to weakness caused by low hemoglobin due to bleeding piles.
3.Ask him to take law salt diet,have more of fruits and salads in diet.Increase content of fiber in diet.Avoid giving foods like pickles,snacks which are high in salt.Regular diet is must.He also needs to regular walking to keep circulation proper.
You can after advise from your doctor give laxatives daily at bed time.Please keep him active and ask to do some regular physical activity to maintain good health.
