Question: Dear Dr.
Good Afternoon.
I have been diagonised Cirrhotic liver disease by Doctor. Scanned copy of Fibroscan showing Median Stiffness 11.4 kPa (F4) is attached. Also prescription of Dr. along with UGS Abdomen, LFT & other blood reports are attached
Kindly go through the reports &prescription/advise of doctor and request your 2nd opinion & advise

Nothing to worry about.

Hello,
Have seen your details and I appreciate your concerns,
I have gone through your reports in detail.
See I would like to tell you that I medical science we have two types of presentation of patient,
1)Clinical presentation- which means what are the symptoms of the patient.To be more easy what are the problems you are having and according to the level of problem you/a patient is having we correlate the investigations done with patients condition.
Now you can see by yourself that every diagnostic test including fibroscan has a disclaimer at end which says that -"Please correlate clinically".
Now due to the advancement of technology over years and change in medical education system rather a drift in system from clinical to diagnostic pattern some times correct interpretation could not be done.
Now sir as per your symptoms details there is no mention that you are having fluid in your abdomen or severe portal pressure or you are unable to digest food etc etc(general function which liver do you are capable of doing them).
Now let's move to diagnostic part.
See fibroscan is nothing but use of ultrasound waves to measure fibrosis(thickening) of liver and fatty changes(amount of fat deposited in your liver.
I know you are most concerned about 11.4 kpa..Now let me tell you a good news sir that highest level is 75 kpa..So your condition overall is a stable patient with cirrhosis.
As far as your liver enzymes are concerned to have a clinically significant disease sir out of all liver parameters atleast 3 of them should be elevated to minimum three times the maximum possible range...In your case liver enzymes are elevated but they are mildly deranged which is normal with cirrhosis as cirrhosis destroys liver cells,Liver cells store liver enzymes and if when these liver cells are destroyed these enzymes goes into blood and blood level get deranged.

As per clinical history s concerned as mentioned on prescription your chief complaint is constipation acid peptic disorder etc.They have nothing significant to do with liver.
You diabetes control is ok ok.
But fasting is a bit elevated(hope 12 hour fasting was done before giving fasting blood sugar).

So overall you are sir a stable case of cirrhosis.
Suggestion is-

Stay away from negativity.
Stay away from doctor who would not judge you by your clinical symptoms but would straight away declare you as a patient of severe cirrhosis just by seeing scan report.
Now let's move to treatment part.
Frankly it's non reversible.
Target- Slowing down cirrhosis rate.
Advice-Modify your life style.
Your meal should consist of complex carbohydrates like wheat,Bajra,Ragi or breads with all of them mixed together to lower the glycemic index and secondly complex carbohydrate would help injured liver to recover...This should be 50% of your diet...plus daily you have to have a small bowl full of vegetables and one bowl full of fruits(catch is less sweet the food the more better it is for you) ,So best ones are apple,guava,and other fiber rich fruits.
Stop dairy.
Saturated fat(any oil which in winter freezes)should be less then 6% and normal oil should be 15% of your diet.
Around 30% should be lean proteins...Take it from all varieties of pulses.
You can also have chicken breast grilled.
Avoid any type of red meat like goat etc.
Take Liv 52 tablet twice daily.
Take sylimarin 800 mg two times a day.
Keep in mind our aim is to live a balanced life as this problem could not be reverted back.
