There is no clearcut guidelines about the use of frequent PEP leads to resistance and becomes ineffective.
I would like to know more about your previous PEP, Whether it was the same with Viropil or with any other drugs before 3 years?
Viropil, which contains the INSTI drug Douletegravir has a high 'genetic barrier' and less likely to develop resistance like other ART dugs. Especially the NNRTI drugs are able to develop resistance earlier which were included in PEP during the yester years.
So, it is very much unlikely to develop resistance to Viropil. Not to worry. Anyhow follow the PEP protocol strictly. (strict adherence to drug, Tests for HIV antibodies on day 0, 1 month, 3 months and 6 months.

Resistance to Viropil is extremely rare.
Imagine that you had taken Viropil thrice previously, That too three years back. Assume these previous exposures exposures were noninfectious episodes (really they were not having any viral particles), then there is no question of viral resistance to ART. Moreover, this time with this present exposure your are not getting any resistant strain at all, if at all that exposure was an infectious one. So once again there is no question of resistance to drugs. Presently your 4th generation HIV test was nonreactive. So you don't have any virus inside your body already. So there is no risk for Drug resistance.
So not to worry. The PEP will be effective 100%.

The HIV antibody test conclusive after 6 months

In any normal situations, the HIV antibody detection test 90 days after the exposure will be conclusive. But as you have taken PEP, the antibody formation (if any) can be delayed up to 6 months. So, in your case the outcome of the antibody test can be considerd confirmative and conclusive 6 months after the exposure.
But PEP taken within 72 hours, (that too within 10 hours as in your case) is a foolproof way to abort the disease if any. Not to worry.

Welcome back.
People are living with daily Viropil for years without any problem. It will not precipitate your pre-diabetic state (Not mentioned the recent blood sugar level (fasting and Postprandial).
If you are already having kidney disease and bad renal function tests, Viropil may worsen the condition. In a normal individual, it will not have any such effect, that too with a PEP (short course). Not to worry at all.
