Question: Respected sir, one of my relative Shri XXXXXXX Kr. XXXXXXX Age 47 has been diagnosed as infected from hepatitis B. After reviewing his report (reports attached)doctor assured him that nothing is serious and he would recovered very soon. One of the major concern shown by doctor is very low haemoglobin level and iron level. Sir, I wish to know that-

1. If any thing serious as per medical report of hepatitis B ?

2. What could be the main reason behind very low level of Haemoglobin and iron level?

3. What would be the ideal way to recover from from problems discussed in point 1 and point 2

Hi XXXXXXX

The hepatitis is likely mild because the liver appears almost normal on elastography (only mild fibrosis) and this is supported if liver enzymes are normal. If he receives medical treatment for the hepatitis, he will likely recover.

In terms of the anemia, it looks like an anemia of chronic illness, e.g. hepatitis. The iron deficiency should however prompt investigations (for malignancy) e.g. by CT do the chest and abdomen and gastroscopy and colonoscopy.

Respected sir, Thanks for your reply. Sir, in the light of your reply some queries are in my mind and I wish to share you for your expert advice-
1.. Sir, as you have suggested investigation for iron deficiency,, if there is a doubt of presence of cancerous elements in his body?
2. What are the reasons for iron deficiency generally in human body? Is it recoverable?
3. Sir in earlier reports, I forgot to mention that he is suffering from chronic piles for last two to three years he looses lots of blood in bathroom while attending nature call. Do this affected haemoglobin and iron deficiency?

Hi again,

Iron deficiency is recoverable if the cause is identified. Here, the most likely cause is intermittent bleeding from piles. If this bleeding is stopped (there are many surgical ways) the anemia and iron deficiency will likely come to an end.
