What is the likely cause of vomiting bright red blood, while passing stool that is black, coagulated blood?
Longer question: My fiancee presented with vomiting black w/red streaks and passing black stool on Friday. I started by treating with oral electrolyte rehydration and monitoring over the weekend (I am a trained Wilderness First Responder). I also asked another specialist here for verification and got a good answer. We continued with oral rehydration over the weekend and it seemed to improve. This morning, the vomiting returned with bright red vomit, and the stool is still black. Her dizziness and weakness has returned. What could be the issue, and is it now time to take her to the ER?

Now its time to proceed for following tests-
CT scan of abdomen to rule out any c0ndition related to liver or portal vein.
Endoscopy(as she is vomiting bright red blood) we have to rule out possibility of bleeding ulcers in stomach and duodenum.When this blood passes through intestines its acted upon by a number of enzymes causing it to coagulate.
Yes it is now time to take her to ER its not just bloody diarrhea.
Go for the suggested tests and follow up with reports.

Thank you Dr. XXXXXXX You have been most helpful. The part that puzzled me was there's no fever, or at most low-grade less than 100 degree F. Pulse and BP remain stable, and I know the bright red vomit localized the issue. I cannot diagnose at home. Thank you for your confirmation, and I will update you.

Thanks for appreciation.
Lets come to your queries-
1)The part that puzzled me was there's no fever, or at most low-grade less than 100 degree F. Pulse and BP remain stable-That's why i am saying that its not just bacterial or protozoal infection.If there is infection in body there is always high grade fever.Most of the bacteria's and viruses can't survive at high temperature therefopre body increases.
If temperature to kill them off.

2)Pulse and BP remain stable-Our body is a complex machinery.If you get dehydrated or loose blood then for few days to maintain blood pressure and pulse body uses hemodynamic phenomenon's(Can't fully explain as it takes almost 40-50 pages in text book to understand compensatory power of body for few days).

3)Move to ER get the tests done and follow up.
