Should I Start To Reduce The Dosage Taking Nexium 40 Mg Twice Daily ?
1) Should I start to reduce the dosage. Try 60 mg nexium daily and then 40 mg.
2)Is it ok to take nexium and pepcid together.
3)I had a colonoscopy done in 2012 and it was clean. How do I know that my gastritis is really crohn's disease ? My two sisters have crohn's disease.
Thank.

Hi,
Thanks for posting your query.
I am Dr.R.K and I am pleased to assist you.
1. You can start tapering the dose. You can take Nexium 40 mg once daily in the morning breakfast and start taking Pepcid in the night. Then after a week you can stop Nexium and take only Pepcid twice a day . And after one week you can stop it.
2. Yes you can take as mentioned above.
3. Crohn's can affect any part of the gastrointestinal tract. But it is common in the terminal part of small bowel.
Colonoscopy may be done to look at the terminal part of small bowel and if any lesion found, biopsy may be done to diagnose Crohn's disease.
To know if the gastritis is due to Crohn's , biopsy may be done.
To evaluate the entire gastrointestinal tract for Crohn's disease, MR enteroclysis, Capsule endoscopy or enteroscopy may be done.

Dr.R.K.
1) When I take 40 mg nexium in morning, do I take 20 mg pepcid in evening or 40 mg. Also is Pepcid AC ok ?
2)You have written that MR enteroclysis, Capsule endoscopy or enteroscopy may be done to evaluate entire gastro track for crohns disease. Will any of these three evaluate the entire gastro tract ? If these are done, then no need for colonoscopy to diagnose crohns disease ? Please explain this part in detail.
Thank you.

Hi,
Thanks for getting back to me with more questions.
1. You can take Pepcid 20 mg in the evening. Yes Pepcid AC is OK
2. Any of the three can be used to evaluate the entire GI tract. MR enterclysis and Capsule endoscopy can only see the lesions. With enteroscopy you can take biopsy to confirm the diagnosis.
3. You can start with colonoscopy. Crohn's disease is common in the terminal small bowel. With colonoscopy you can access the terminal small bowel and look for lesions of Crohn's disease and take biopsy if lesions are found.

Dr.R.K.
