As per data 16 to 17 cm is normal size of liver. So size 18 in a person will be called mild increase in liver size.
Detailed history makes it possible to have judgment about the proper clinical evaluation and further specific investigations to know the cause. Blood tests for CBC, diabetes, kidney, liver, pancreas and thyroid functions.
However, as you are having constant pain with long term fatigue I would suggest you to undergo some cardiac investigations initially like 2d echo EKG and trop to rule out unstable angina or an ongoing heart event. Sometimes when a particular blood vessel is clogged up patients heart can't supply the amount of blood required to the body hence person feels fatigued.
