Since 1 Month My Sister Is Not Well She Is
Question: Since 1 month my Sister is not well she is having fever of 101 every night from 8 days then i admitted her to hospital after rulling out all the possiblities doctor ordered her for usg abdominal the finding are she is having mild acsites on abdominal and sub centimeter swollen lymph nodes of the abdominal and doctor then advised for montux and her montux become positive then doctor advised her antituberclosis now its been 5th day of the treatment now she dont have any fever but stills she is unable to eat anything even she hardly drinks water too ..
My questions are following
1. Does this treatment is good for her?
2. How many days it will take for her to become normal again with this ATT?
3. If she vommits soon after 15 mins of taking any med should i have to give those meds again on the same time?
4. If she vommit does any chances of treatement not to works??
5. any of your advised welcomed..

Hi,
I went through the details. The current evidences suggest possible tuberculosis infection and it is appropriate to start antitubercular medications. However it would be a better to get a CT abdomen. CT / MRI scan is a better investigating tool then USG as it the former is more useful to study the size and morphology of lymph nodes. You can discuss with the treating doctor.
Tuberculosis treatment is a long duration treatment unlike any other common infection. Extrapulmonary tuberculosis like this one may need upo 9 months of anititubercular drugs.
AKT medications is a combination of 3 or 4 medicines. One of it especially rifampicin is known to induce gastritis and vomiting, May be the same is happening with your sister. Over a period of days, most patients would start to tolerate it well. Until then if she vomiting, it may be worthwhile to start proton pump inhibitor drugs + domperidone. You can check with her treating doctor.
And yes, if she vomits soon after medications, it is better to repeat the medications.
The only other advice I give to your sister is stay healthy. Eat nutritious food and involve in mild exercise.

sir tell me what to do if see vommits after 20 mins of taking med should i repeat the dose? and please tell me in how many days she will became normal and symptom free again..?
madam advised tab PZA 500 BD twice daily along with other meds today after 20 mins of the PZA 50 BD (evening dose) she vommits should i repeat the evening dose or should i wait for morning.. ?? pls guide asap
and also pls tell me that does abdomibal tuberclosis is contagious should we have to take precaution while handling her should we cover our nose while she sneazes, coughs, or vommits can we share same toilet seat please tell me is any chance of contratcting to tuberclosis??

It is important to have medications regularly for effective treatment of tuberculosis. Vomiting within 20 minutes might impair full absorption of medications. I would suggest to repeat the dose. But in order to prevent further vomiting, it is advisable to use antiemetic drugs.
I would start tab pantoprazole 20mg + ondansetron / domperidone twice daily for a few days until she gets used to the medications. All other antitubercular drugs including PZA can be taken thereafter. Discuss with her treating doctor.
