Question: hi doctor. i am a 44 year old male with a high coronary calcium score of 98 which represents the 96th percentile for age and gender. and that puts me at risk of cardoovascular event. i did an extra test which is the stress echo test which revealed excellent results. since the 2 tests are contradicting what should i do ? perform an angioplasty test ? am i at high risk now ? thanks

Being in the calcium score range of 11 - 100 means that a moderate probability for coronary event is present. Coming to this point, it is necessary to quit smoking, follow a healthy diet profile (preferably a Mediterranean diet), and thinking to begin an exercise program; following necessary treatment in case of dyslipidemia and hypertension.

All the above measures would help to reduce any theoretical probability of developing coronary events in the next 10 years.

Though such a calcium score classify you as having a mild heart disease and a moderate risk for future 10 years coronary events, this approximation holds true in case the individual in case of a moderate coronary risk (presence of certain coronary risk factors, such as hypertension, smoking contact, diabetes, dyslipidemia, etc.)

In such case, changing the risk profile by controlling the above mentioned factors (properly treating hypertension, diabetes, dyslipidemia, avoiding close smoking contacts, etc.) will lead to a reduction of the theoretical future risk.

many thanks doctor ! appreciate your detailed feedback. actually i am under lipitor 20 treatment to slow down the progress of the disease. i have no other risk factors like hypertension or diabetes etc. and dont smoke at all and exercise 1 hour per day. should i take coenzime q10 with lipitor ? how about taking aspirin as well ? many thanks !

I understand your concern and agree with you on the fact that lipitor may decrease Coenzyme Q10 levels.

For this reason, taking Coenzyme Q10 would be a reasonable treatment choice.

Regarding Aspirin, you should know that different studies have shown controversial conclusions about its benefits for primary prevention, when there is no evidence of coronary artery disease or stroke.

For this reason, you can take Aspirin if you have no contraindications on it, but as your cardiac stress echo is excellent and you have no symptoms of angina, I don't think that it would make a change in your cardio-vascular risk.
