Sir, My Wife Suffering From Chronic Piles From A Long Along With Anal Fissure Our Concern Is There We Do The Surgery For Piles And Now The Fissure Is There So Again We Need The Surgery Or Through Some Medication It Can Treat?
My wife suffering from chronic piles from a long time and based on doctor advice we do the surgery for piles in month XXXXXXX this year after that everything goes fine,But from last 10 days she got pain and heavy bleeding we consult the same doctor who done the surgery. After examine he told that Soiling is there and along with anal fissure is also there which he told not more than 5%.Can you help out how the fissure is developed and why still pain and soiling is there.He give medicinefor pain killer and bleeding. Our concern is there we do the surgery for piles and now the fissure is there so againg we need the surgery or through some medication it can treat.

Soiling is involuntary passing of fecal matter. It could be due to fissure.
Anal fissure can co-exist with piles. After treatment of piles it may have become more severe. Fissure usually has severe pain, bleeding with bowels and due to pain soiling maybe there.
Fissure is usually treated with medication initially. If it still persists or prolapsed pilles develop, then surgery maybe needed. Do apply lots of pain killer cream and vaseline all over (and little bit inside) the anal area whenever needed especially before bowels. Drink enough of water (clean water) so that stools are soft. Ghee and butter in diet can add some grease to the stools so that they pass out easily.

Basically there should be enough of pain relief and greasing in the area especially during defecation.
Nitrogesic cream is good enough for healing. Also you can have lignocaine gel (anaesthetic) to apply before defecation. I personally like vaseline but coconut oil can also be tried in its place.
