Brief Answer:
Needs interventions from a clinical psychologist

Detailed Answer:
Thanks for using the Ask a Doctor service.

I have gone carefully through your query. I understand that the child was born preterm. However, that effect is unlikely to last till date unless there were definite signs of hypoxia (lack of oxygenation) around the time of birth. Nevertheless, I would insist you to visit a child psychologist and to get a psychometric evaluation done. The psychologist would be able to look up signs of ADHD which may lead to decreased attention span which in turn could lead to faltering performance. His inferiority complex also needs to be dealt with and the psychologist would be able to do the needful.

Please do not ignore. Act at the earliest.
