Question: Sir,Mam
I was in Bangkok yesterday visiting few of the bars. The bars had comfort women who will sit with you over drinks. My interaction with the women were
1. No Vaginal Or Anal Or Oral Sex
2. No Lip to Lip Kissing
3. The lady would sit on my lap and gyrate on music. .
She would use her b and move over my penis. I was
clothed all the while. She was wearing an
underwear and on top of the underwear she was
wearing a swim suit.
4. The women kissed me on my cheeks
5. I too kissed them on the cheeks, neck and
cleaveage.
6. I also played with the breasts and kissed them.
7. The women gave me a love bite behind my neck.
There was an abrasion and very minute bleeding.
8. I also fingered her private part over her pants and underwear never touching the private area. But I could feel the wetness in my hand. Accidentally i used the same fingers to rub my nose
Given the above would you consider any risks of HIV or any STD.

1. When you use negligible you do you mean there is some chance of any sort of transmission. From my reading so far most of the STD are found in the bodily fluids in the cervix region. Given that there has been no penetration shall i conclude this to be a non risk.
2. As I have also mentioned previously that upon fingering the woman through her dress my fingers became wet and i rubbed my nose with the same. Does this pose any issue
3. Do i need to get tested for this encounter and do i need to refrain myself from interaction with my ususal partner. This is my only exposure for now and i have recently tested for everythi g as a requirement for my foreign work permit

Medicine is not mathematics to constant answer.
You had received a love bite over the neck. If any lesion in the mouth, then there is slight risk.
You had become wet with vaginal fluid? and rubbed your nose... If you had an microscopical abrasion either over your finger or nose without your knowledge, then we can not rule out the little risk.
So only I used the word "negligible".. It means very very little chance, say,less than
0. 2%. We can not use the word that your risk is nil. Hope you understand your situation.
Regarding the test, If you would be in your position, I would prefer to go for the tests before I resume my sexual activities with my usual partner at least for the sake to overcome the unnecessary mental tension.

I am within 72 hours of starting PEP. Do I need to go for it.
Another point is the bite abrasion was more of a scratch and not a deep wound. The lady was drinking with me at the time so I would think if she had mouth lesions she would not be drinking.
Secondly the rubbing of nose after fingers wet was not immediate it was say after a minute or so, would it not become inactive when exposed to nature elements

Lastly when is the latest I can test. Is it at 28 days

Abrasion need not to be deeper to make an entry,as virus are ultra smallest particles.
Chances of virus in body fluid (vaginal wetness) could be viable as long as the wetness was there. If it got dried it will die. If you are sure that the wetness dried completely before you touched your nose, even then it would depend upon your finger status.
If you had started PEP, continue the same and finish the course as suggested by your physician and then you can forget about the least possibility of HIV infection.

Sir I understand one of the drawbacks of online consulting is you do not see the patient in person and I appreciate your feedback on the basis of written material. Would you have recommended PEP had I come in person to you. I did not do PEP as I am not so medically informed as a doctor can be.
1. The lady who bit me was also drinking alcohol with me so I would reasonably guess if she had mouth ulcers or lesions it would have been difficult for her to drink.
2. When i use the word abrasion, i mean it was like a minor skin scratch like u get when u get grazed. There was no skin indentation or oozing of blood from a wound. It got healed itself quickly
3. On fingering let me clarify there was no direct contact of my finger with the private part as as a result no direct contact with the fluid from the source. The fingering was over the undergarment and the jeans short the lady was wearing. I could feel the wetness in my finger though. Also I did not touch my nose immediately as I recall rubbing my fore finger with my thumb to sort of clean away the wetness before rubbing the nose. I would reasonably believe my hand was dry.
I appreciate your feedback that the risk is negligible. I would see a doctor today as the clinics are closed for holidays here. This is my last question. Thank You and May the almighty bless you for the good work you are doing
