So I Am A 42 Year Old White Male That

Question: So I am a 42 year old white Male that has been having issues for the past 4-5 months, with pain in my outer legs (IT Bands) Spine pain, bladder urgency and discomfort, and painful ejaculation. Everything has been cleared medically, Lower Lumbar MRI, Lower abdomen CT, blood-work, treated with antibiotics, seen urologist, and gastrointestinal doctor. No blood in stools, prostate checks out fine.I have no insurance so I am paying out of pocket. So here is the thing. I am closet bisexual even though I have never been with a guy, so Idk how that works out verbally. Anyway so occasional I will mess around with some things at the house with anal insertion when no one is home. Nothing crazy or big, not into any S&M or anything. Obviously I am terrified to tell anyone this so no...none of my doctors know this information following. So I am always clean, before I do this and make sure what I am using is clean. Its actually a little handle to a brush. I make sure there is plenty of lubrication, and it is made out of a hard plastic. So I would position my self like a "Top" and insert this anally and proceed as if I were riding another male. The handle is about 6" long, it is rounded at the end, no sharp or rigid points, smooth, but not flexible. I have done it probably a dozen times. So now for about 4-5 months I have developed all this pain in my hips, almost a hyperalgesia in my hips. Painful ejaculations, bladder issues, really bad pain in my outer leg bands, tingling feelings in my inside thigh. Also a pressure like if someone were to describe BHP to you, except I don't have that. Is there a possibility that I have injured something inside of me? I mean there was never an incident with any blood or violent insertion, but could i have gone too far and damaged something? I am a relatively skinny guy and this thing is about 6 inches long maybe a little longer, nothing overly big in diameter. I am so embarrassed that I have to get online and ask this I dont know what to do. I currently gave a gf that I cannot talk with this about for obvious reasons. If any of you have any thoughts on this please please help. This is excruciating pain and it never goes away, day night, I take Ibuprofen, tylenol, nothing really helps. It worsens when standing and it feels like pressure like my hips are trying to crack open in the back. no problem passing urine or bowels although my urine stream is not as strong as normal. pain not worsened by either. Please help or if you want to ask me anything else concerning this, please ask. Thank you for listening.

Also this is probably a far stretch, but I have also had Shingles/Herpes zoster in my eyes and face before and also have HVS2 since I was 18. I have read some cases where that virus can infect and inflame the L5 and give multiple symptoms, just forgot to mention that.

Brief Answer:
sacral ganglia or other nerves involvement through HSV 2.

Detailed Answer:
Hello and welcome to Ask A Doctor services.
I have read your query and here is my advise.

* As per my surgical experience, the most sounding possible cause of your
painful ejaculation and associated symptoms indicate likely involvement of
sacral ganglia or other nerves as latent effect through previous HSV 2
infection.
And most of these cases will not show up on MRI as there is no obvious
changes to be seen over radiology imaging.

* The foreign body insertion with proper lubrication is very less likely to
provoke such episodes.

* The MRI and urologist with rest work up I have gone through, but I would
like to ask about further following details so that I can answer your query
more precisely.

1. Give details of bladder urgency and dysfunction.
2. What about erection - is it painful or discomfort ?
Type of pain while ejaculation ?
3. What about sensation over lower back skin, thigh, legs ?
4. History of diabetes, high BP or other systemic illness ?
5. History of smoking ?
6. Any neurologist opinion taken ?

Awaiting for your response.
Regards.

1. Give details of bladder urgency and dysfunction.
It feels like a light UTI or a prostate infection. Just pressure in the parineum. I feel like I have to pee, then I go, and dont feel like I went all the way. Not really excruciating pain there.
2. What about erection - is it painful or discomfort ?
Erection is not painful, its different though.
Type of pain while ejaculation ? I can have sex or masturbate and it feels fairly normal when I ejaculate but about 15 minutes afterwards, it will become very sore (pressure intensifies) down there.
3. What about sensation over lower back skin, thigh, legs ? Here in the past week, my skin on the outside of my hips and back have become VERY sensitive to pain, even clothing...touching me. VERY painful to any contact sitting or otherwise pressing on the skin.
4. History of diabetes, high BP or other systemic illness ? No no others, migraine headaches when I was younger.
5. History of smoking ? I smoke Weed now but in home calmly. Same source for years. Used to smoke cigs when I was younger off and on but never constantly or for long.
6. Any neurologist opinion taken ? Not yet.

Thank you so much I do appreciate your feedback.

Brief Answer:
need neurology opinion and QUIT weeds dear.

Detailed Answer:
Hi
Thanks for the relevant details in a very precise manner which actually helps great way to know the issue in depth.

* So I would like to continue with my possibility of neuronal issue there with
further details.

* A neurologic examination with expert neurologist is mandatory from this
point of view as merely taking pain killers will not solve the issue at all.
He or she will conduct detailed physical examination from neurology point
of view and along with that specific tests as nerve conduction study , PCR
( Polymerase chain reaction ) or else for the viral etiology work up.

* As the issue of bit long period, evaluation process will take a precise time
and you have to opt with patience till the diagnosis and further
management plans.

* You would not like to read this, but it is fact that by consuming Weeds you
are harming your neurons and nerves further with false assurance of
transient pleasure. ( There are plenty of medical reasons to quote this
statement, one of major is effects over the nerve transmission, neuronal
transmitters and others )
So should think of seriously to quit if you want really healthy life ahead
dear.

Wish you great day.
Would like to assist further as per your need.

Thank you.
Regards.
