Question: So over the last year I have started feeling tired throughout the day, irregardless of the amount of sleep I get. Sometimes I’ll have a lie in and still require a nap later on. I find myself with no energy to do anything and lack of desire to go out and do things. Because of this I’ve since stopped going to the gym and other activities I once did. Also I find I’m extremely forgetful day to day, and I struggle to focus. Focus is a big issue as people will tell me things and I won’t be listening, or I struggle to watch an entire film as I can’t focus on it. I struggle to keep organised because of this inability to focus. As a side note possibly unrelated but I also experience twitches throughout my whole body, which become bit jerks at night. Not painful just annoying as my job is on a compute and sometimes my hand twitches on the mouse. Just wondering what is happening as it’s frustrating.

Possibilities include-
1. Thyroid hormone dysfunction, which can cause forgetfulness and tiredness.

2. Vitamin B12 deficiency can also cause several of these symptoms.

3. A brain scan should be done to exclude secondary causes related to various brain diseases.

4. Muscle twitching are most likely due to benign fasciculations. However, if doubt persists, an EMG may be done to rule out other causes.

i have had multiple blood tests whoch have all shown fine. They looked at my b12 and thyroxine levels and they were not abnormal, so not sure what to do.

Blood tests for B12 and thyroxine are first level tests, which are normal.

However, since your symptoms are persiting, you should proceed for the second level work up. These would include:

1. Neurological clinical examination

2. Brain scan, preferably MRI,

3. EMG study
