Question: Some background history: I am a 32 year old female. I am 5’2” and 135 pounds. I have tested positive for anti thyroid peroxidase and have been diagnosed with Hashimoto’s 1.5 years ago. Although my thyroid levels are within range, 1 week ago my doctor started me on Levothyroxine 12.5mg a day. I still have all my wisdom teeth because they’re too close to the nerve to remove.
I have had 3 colds from October 2020 to December 2020, all without fever. After the last cold in early December, my sore throat never went away completely. There is redness on the palatoglossal arch on both sides of the uvula. I had an ultrasound of thyroid and it appears to be normal. In early January I had about 3 weeks of a lump in the throat feeling along with the sore throat. Then about 3 weeks with a sore throat only. Now again I have a lump in the throat feeling and the sore throat has never stopped completely. In December I began to feel intermittent pain in or around my ears, particularly my left ear. It is shooting or sharp and mild. The doctor said the ears are clear and look healthy. Also, early January I began to experience mild discomfort on the top left side along the teeth. This discomfort progressed to mild pain fluctuating between the top left teeth and the bottom left teeth. The dentist found 5 teeth with cavities of which one tooth is on the top left and one on the bottom left. None reach the pulp so should not cause any pain. The dental exam was short and not thorough. Shortly after, I began to experience soreness in my entire top and bottom of the jaw and around the ears. The mild soreness progressed to extreme soreness and pain with severe headaches or migraines almost daily. My jaw feels so tired. It is hard for me to relax my jaw. I don’t even know how to position my jaw and my teeth to relax. It is as though my entire head is in pain now.
Long story short: sore throat with redness, intermittent ear pain, jaw/mouth pain and soreness, headaches, intermittent lump in throat.

Hii. Welcome to the healthcare services. It seems that you are having acid reflux, allergic pharyngitis and allergic inflammation of the nose and Temporomandibular joint arthritis. Start with warm saline gargle alternate with betadine gargle. Take steam inhalation through nose and mouth. Start nasoclear nose drop twice daily for 2 week. Take Pantoprazole 40 mg once daily before food for 2 week. Drink more water . Avoid spicy food, excessive tea, coffee. Give hot fomentation over the jaws. Your tonsils are congested,you are having headache with lump sensation of throat. All these indicate to allergic inflammation and acid reflux. So lead a healthy life,take steam inhalation,hot fomentation. Please review if you have any queries.

I don’t understand how I can have acid reflux if I have no cough, no burning sensation, and no clearing of my throat. Also, my throat is not sore in the back. It is sore, irritated, red in the front part of the throat opening next to the uvula. March 5th will be 3 months since I have a sore throat. I moved to a different area in September, but my sore throat started 2.5 months after moving so I don’t think that is the cause. Do you really suspect I have acid reflux? Also, if I am having allergic inflammation of the nose and temporomandibular joint arthritis, after I stop the nose spray will the inflammation return? Am I doing something to congest my tonsils and cause inflammation? Thank you.

Hii. It can be a combined effect of allergic rhinitis, allergic pharyngitis and acid reflux. So start with betadine gargle alternate with warm saline gargle. If your throat swab is persisting then take a throat swab and sent for culture and sensitivity. Wait for the report. If the report shows growth of some organism then antibiotic need to be started according to the report. Avoid excess tea,coffee, spicy food. Take steam inhalation. Give hot fomentation over the jaw. Continue the spray for 1 month. Symptoms may appear after 1 month but steam inhalation,hot fomentation and saline gargle will help u.
