Some Times I Get Needle Like Pain Inside The Head

Question: Some times I get needle like pain inside the head of penis. I'm uncircumcised. I'm married. Sometime when I masturbate and foreskin moves around the opening it pains till the time its dry. Also the two sides of penis opening aren't stuck.. They open wide like a fish mouth. I had unprotected sex with a woman who's not my wife. there's itching inside my foreskin somewere around the head of penis. It itches for a couples of seconds and vanishes. No burning sensation in urine when I have more water. But it urine is in yellow color, when water intake is low, there's little burning sensation. Once urine passes there's relief from everything. It's been 5 weeks and 2 days since tht sexual encounter. There's mild pain in pubic region and left testicle nerve. At tyms I'm not able to understand that whether pain is in pubic region or top of testicle nerves. There's no pain during masturbation or urine passage. The sexual encounter was with a newly made friend and not a sex worker. Also to note when I got newly married and had sex with wife I used to get mild itching. Have taken no treatment or medicine yet. There are no sores, fever, flu, no loss of diet, no headache or backache, no fatigue.
