Question: Sometimes my right testicle feels out of place until I turn it back to a normal vertical position. I think I’ve always been this way but I’m not sure if this is just a normal re-adjustment or if I have Bell Clapper Deformity. For example, this morning when I woke up, the testicle was positioned almost horizontal (pointed outward toward the wall) and I wasn’t entirely sure which way to turn it so that it was correct but I put it back in a vertical position and it feels fine. I think what I did would be considered close to a 90 degree turn or 1/4th of a full circle.
My questions are:
1 - Is this normal or does it sound like Bell Clapper Deformity?
2 - From twisting it, could I have a testicular torsion? I don’t have any pain. It feels like it’s in the correct vertical position.
3 - Coincidentally, about a month ago I had a testicular ultrasound, because I was worried about one testicle being a lot larger than the other. The ultrasound came back normal. Would it have noticed Bell Clapper Deformity?
4 – My girlfriend is coming over for the weekend. Do I need to worry about this when being physically intimate with her?

I just wanted to add that it did stay pointing horizontal (towards the wall) even once I stood up. But I then put it in a vertical position and it feels fine.

What you describe certainly sounds like Bell Clapper. It would not necessarily show on ultrasound. It increases the risk of torsion, but if you do not feel any pain now, you do not have torsion. If it becomes a recurrent problem with the pain, you could have a minor surgical procedure performed to fixate the testicle in your scrotum. There is no problem on being intimate with your girlfriend.

1 - Is there any way to know if the testicle is now in the right place rather than either backwards or upside down? When I turned it back to vertical this morning, I wasn't sure which way to turn it. Also, it feels like there is a small cord towards the front bottom of the testicle at times although I thought that was supposed to be in the back. However, I think that has always been more towards the front.
2- If the testicle was upside down or backwards when the ultrasound was taken, would it have shown then? I'm asking because I think the cord was in the same spot then.
3 - I'm kind of afraid to go back to sleep because I'm afraid it's going to turn sideways and I might accidentally turn it the wrong way. What should I do? I would like to see a urologist but I doubt I could be seen before May because in my area, non-urgent medical appointments are being done virtually. Thank you for your help.

OK thanks. Sorry to keep bothering you but I just noticed something else. It feels like there is something firm about the length of a pea but maybe half as thin (maybe 3 mm) towards the top of the right testicle. I don't know if it's actually a growth on the testicle. It feels like it's a little above the testicle and in the scrotum but I'm not certain. Sometimes if I try to move it, it feels like it's connected to a tube. Is that just the epididymis (maybe with a varocele on top?)? I'm a little worried it could be a cancerous growth. I don't feel anything this size on the other testicle and I didn't used to feel it on this testicle. I don't know if this testicle is just in a different position than it used to be though and that's why I'm feeling it now. I think overall what I'm feeling is smaller than a pea (mainly because it's thinner).
I also feel something at the bottom that is a lot thinner (maybe 1 mm). I don't know if that's the tail end of the epididymis or a vein or something else. I hope that if any of this was abnormal, it would have shown in the ultrasound 2 months ago unless this just developed now. Does any of this sound abnormal?

Last night, I kept touching that small bump and trying to see if it was moveable (my understanding is that firm growths that don’t move in the body are more likely to be malignant). This morning, when I was going to masturbate, it seemed like the top of the testicle was a little hypersensitive to touch and positioned a bit higher and more inward in the scrotum than usual. I masturbated about an hour ago but it still feels higher and more inward than usual and a little larger and tighter. When I sat down earlier, for a moment it felt sensitive too. Does this still not sound like I have a malignant growth and not sound like it’s anything else urgent? The right testicle has actually always felt a lot larger than the left even though the ultrasound found only a small difference.

Is it extremely unlikely that I developed a very tiny cancerous growth after my January 30th ultrasound (or that it was too tiny to show up on the ultrasound)? It does still feel small (about the height of a pea and half the thinness of one). And it still doesn’t sound like a testicular torsion?

I also wonder if all of this is normal and I’m just anxiously messing around with and monitoring everything in the area. I partly want to go back to my doctor to have it looked at but I don’t think doctors are scheduling in-person visits until at least next month because of covid 19 except for urgent appointments. (And I’d worry about catching covid 19 if I went to a doctor’s office). I appreciate your feedback.
