Spouse Had Recent MRI For Severe Dizziness , Double Vision
They called it a negative MRI for stoke. And most Likley symptoms are vestibular in nature. My question is could these findings represent PML? She is on rituximab, azathioprine and corticosteroids for polymyositis. She tested positive for the JC virus with a relatively high tiger prior to treatment with rituximab. Just don’t want to miss anything.
Unfortunately PML is a possibility in this case.

I have read your query and here is my advice.
PML is possible in persons who have a weakened immune system and infected with JC virus.
So, I must say that in your spouse's case, this is possible since she is using immunosuppressive drugs, resulted positive for JC virus and there were white matter changes in the MRI.
The necessary tests to evaluate correctly PML and to exclude it are test of cerebrospinal fluid for JC virus DNA and brain ( lesion ) biopsy that can be drawn under stereotactic guide.
Discuss with your spouse's Neurologist for these issues.
