Suggest Alternative Medication For Scalding

Question: my 6 months old niece got hurt by boiled oil on her right leg, the burnt skin peeled from the effected area, we have applying the silverex cream from two days, but there is no much result. we are worrying about her, please suggest to get it recover soon.

Thanks for the query

The scald due to boiled oil with peeling of skin is usually second degree burn.With proper treatment it heals completely without any scar but it may take 2-3 weeks to heal it completely.
The main aim is to prevent secondary infection.
I would suggest you to continue application of silverex cream.Clean your hand properly before applying this cream.You can also apply mupirocin cream to prevent secondary infection.
I wont get worried if it is not enlarging and there is no pus formation.
You can give ibuprofen syrup for pain.
I request you to attach an image to assess the extent and degree of burn.

Dear Doctor,

Thanks for your reply,

Please find attached images.

2-3 weeks is too long, how can a kid can bare this pain so long, as i said we are using silverex at this moment, How effect this cream works?, is this also works as antibiotic?

As you mentioned can also apply mupirocin, this can apply along with silverex cream, else have to use any one of them.

If we use ibuprofen syrup what is quantity that we have to give, How this works?

There is problem with her that she always shakes her legs, she swipes the cream with another leg, it wont stay much time.

I know i do not worry much about it, but she cries and not able to sleep caz of this, pleas suggest your best.

Thanks for writing back.

I have gone through the image.The size of lesion is not that large as I had assumed.
Silverex contains silver sulphadiazine and chlorhexidine. Silver sulphadiazine has a good antibiotic property and it works well in preventing bacterial infection in burn lesion.
I advised you to apply mupirocin ointment assuming that the lesion is extensive(for better coverage of gram positive bacteria).
But after going through the picture I would like to continue Silverex only.
Your another concern is regarding duration for healing.
There was some misunderstanding and I am sorry as I have not mentioned it clearly that the 2-3 week is the time duration for complete healing.
The scab will form within 5-7 days and after that the lesion will be painless.Also the chance of infection is very low after 7 days.
So for next four to five days you need to give Ibuprofen which is an analgesic.
The dose of ibuprofen is 10-15 mg/kg/dose two to three times a day.
5 ml of ibuprofen syrup usually contain 100 mg.So assuming his weight about 6 kg at 6 month of age 6-7.5 ml of ibuprofen will do.
It is not necessary for the ointment to be there through all the time.After application within 15-20 minutes most of the component of antibiotic get absorbed and which remain effective for next 8-12 hours.So don't bother about the duration of stay of ointment at the lesion site.
