Once anal fissures get chronic, they are very much difficult to heal by medication alone. As part of conservative treatment topical metronidazole, topical steroids and local anesthetics like lidocaine are used. Systemic antibiotics with anaerobe coverage are also prescribed to help improve healing. Constipation should be avoided by using stool softeners, ispaghula husk and cremaffin syrup. A lateral internal sphincterotomy where a part of the anal fissure is removed in order to relax the sphincter. It is the treatment for such a case to be decided in consult with a General surgeon or Proctologist.
