Brief Answer:
Better to stop nortryptiline gradually

Detailed Answer:
Hello,

Thanks for writing to us.

I recommend reducing 25 mg of nortryptiline every third day and stop. When nortryptiline is stopped cold turkey withdrawal features are possible so slow tapering is better.

You can start cymbalta when dose of nortryptiline reaches to 25 mg as cymbalta can increase level of nortrytiline by affecting its metabolism.

Inform above recommendations to your treating doctor and change dose under his/her supervision.

Hope I have answered your query, I will be happy to help further.
Regards,
Dr.Chintan Solanki.
