Most important part of treatment of anal fissure is to keep stool soft by taking laxatives and lots of fibres and fluid in your diet.
Most recommended laxative is polyethylene glycol. It comes in a XXXXXXX form containing 13.8gm in each XXXXXXX To be taken after dissolving it in a glass of water at night time. Titrate the dose according to the response
Psyllium fibres also to be taken at night time to make stool bulky and soft
Apply local ointment containing lignocaine, and diltizem per annum three to 4 times in a day
Do sitz bath 3-4 times in a day
Take calcium dobesilate 500 tds
All above medicines are available OTC.
Do all this for around 10 days or according to the response

Diltiact ointment and Xylocaine jelly are the brand name of ointment containing diltizem and lignocaine respectively. Both of these are to be applied per annum thrice to four times in a day especially before and after passing stool
Dulcoflex tablet is also suitable it contains dulcolax, however in some it may cause abdominal cramps.
