Suggest Medication For Anxiety

Question: Hello Dr
My spouse (age 37years) is suffering from Anxiety, Fear, Headache since 3years. We consulted a doctor for the same on 08th Aug'16. Since then he gave medicine which are attached herewith. But the problem still persists. we consulted doctor every month and he continued the almost same medicine. Pl have a look on the problems and prescription. Pl suggest the best advice to get out of problem.

Have you received two prescriptions which I attached herewith. As far as Symptoms are concerned, these are headaches, Ghabrahat, how can I take of my children, frustration, like to sleep on bed all the time.
